After you register with My Aged Care, they will arrange an assessment to understand your needs. There are two types:

Home Support Assessments: This is for basic support at home (like cleaning or meals) through the Commonwealth Home Support Programme (CHSP).

ACAT (Aged Care Assessment Team): This is a more comprehensive assessment for more complex needs, including eligibility for a Home Care Package or for moving into a residential aged care home (nursing home).

This is a comprehensive annual health check-up available under Medicare for all patients aged 75 and over. It’s an excellent opportunity for the doctors to proactively review your overall health, including your memory, mobility, medication, and home safety. It also helps identify any potential issues early and plan for your future health needs.

Yes. Managing many medications can be confusing. Your GP can conduct a thorough medication review. They can also arrange a Home Medicines Review (HMR), where an accredited pharmacist visits you at home to help you understand your medications and ensure you’re taking them safely and effectively.

These are important legal documents that allow you to appoint a trusted person to make decisions on your behalf if you are no longer able to.

Enduring Power of Attorney covers financial and legal decisions.

Enduring Guardianship covers health, lifestyle, and medical decisions.  The doctors at Healthfront Doctors strongly advise all their older patients to put these in place.

An Advanced Care Directive is a formal way to record your wishes for your future health and medical care. It’s a guide for your family, doctors, and your Enduring Guardian to ensure your treatment aligns with your values, especially for end-of-life care.
